The CDC has a vaccine injury and death reporting system called VAERS. According to the CDC, less than 1% of vaccine events are reported in their system. Doctors are encouraged to report any vaccine-related events to this system, however, most do not know it exists.
All reports are searchable to the general public and can be downloaded.

The Death Count (USA Only)
According to a Harvard study, less than 1% of all vaccine deaths and injuries are reported to the CDC's Vaccine Adverse Event Reporting System (VAERS). We have every reason to believe that the CDC is also hiding data.
